To access the Hospice Hospitalization and Care service, you must:

• Have a medical prescription from your doctor indicating the need for palliative care.
• Contact the Hospice or the Local Health Authority's Palliative Care Service for information and reservations.
• Provide medical documentation relating to your illness and ongoing treatments.
• Assessment by the Hospice medical team to confirm admission and plan your admission.
• Reception and admission are subject to availability and clinical priorities.
