How to request and collect a certified copy of the documentation
The service is intended for those entitled to request a copy of their medical records or clinical documentation, in particular:
The request can be submitted after discharge to the Medical Records Office of the referring hospital or another competent office, according to the procedures established by the Health Authority.
In all cases, documentation is released in compliance with current legislation regarding privacy and personal data protection.

If submitting by mail or fax, the request must be accompanied by a copy of a valid ID of the record holder or the person authorized to make the request.
The medical record is an original document owned by the Health Authority: for this reason, it is issued only as a certified copy, not as an original.

How much does it cost?
The release of documentation is subject to the payment of reproduction costs and any shipping costs.

How long does it take?
Documentation is issued within 7 days of the request or the file being made available to the Health Department. Urgent requests are only accepted for proven medical needs.
