How to request and collect a certified copy of the documentation

The service is intended for those entitled to request a copy of their medical records or clinical documentation, in particular:

  • to the interested insured person, after discharge, upon presentation of a signed request accompanied by a valid identity document;
  • to the minor's parent or guardian, upon presentation of the necessary documentation demonstrating their entitlement to the request;
  • to the emancipated minor, who may submit the request directly, attaching appropriate documentation;
  • to the guardian or curator of the incapacitated or incapacitated insured person, with appropriate certification attesting to their title;
  • to the legitimate heirs of the deceased insured person, upon presentation of documentation or self-certification attesting their status as heir and their relationship with the deceased.

The request can be submitted after discharge to the Medical Records Office of the referring hospital or another competent office, according to the procedures established by the Health Authority.

In all cases, documentation is released in compliance with current legislation regarding privacy and personal data protection.

Come fare

The request can be submitted:

  • directly to the Medical Records Office;
  • by mail;
  • by fax.

If submitting by mail or fax, the request must be accompanied by a copy of a valid ID of the record holder or the person authorized to make the request.

The medical record is an original document owned by the Health Authority: for this reason, it is issued only as a certified copy, not as an original.

Who can collect it

The copy of the medical record may be collected by the applicant or a delegate, provided with:

  • a signed authorization;
  • a copy of the delegating party's valid identification document;
  • a ​​valid identification document.

In special cases, upon express request of the interested party, collection may be permitted only to the interested party.

How much does it cost?

The release of documentation is subject to the payment of reproduction costs and any shipping costs.

Fees:

  • Emergency Room Report: €5.00
  • Outpatient Record/OBI: €10.00
  • Simple copy of medical records: €15.00
  • Certified copy of medical records: €20.00

Payment must be made at the time of request, via self-service machines or postal order.
Additional charges apply for mailing.

How long does it take?

Documentation is issued within 7 days of the request or the file being made available to the Health Department. Urgent requests are only accepted for proven medical needs.
