Integrated Home Care (ADI)

Cosa serve

To request activation of the ADI service, you must:

1. Contact your General Practitioner (GP) or Pediatrician of your choice (PLS).
2. Obtain a prescription and report it to the local health authority (ASL) services.
3. Submit the request at the Single Access Point (PUA) of your local health district.

Alternatively, the request can be activated:

• During protected hospital discharge.
• Upon referral from the municipal social services.

After the request:

• The patient is assessed by the Multidimensional Assessment Unit (UVM).
• An Individualized Care Plan (PAI) is developed.
• The type, frequency, and duration of interventions (medical, nursing, physiotherapy, social care) are established.

Generally required documents:

• Identity document and health card
• Updated medical records, if any
• Prescription from the attending physician

Tempi di attesa

Activation times vary based on clinical priority:

• Urgent: within 48–72 hours
• Regular: within 7 working days of assessment
• Scheduled: subject to availability of the local service

Timeframes may vary based on the district's healthcare load.

Costi

The ADI service is free for eligible citizens, as it falls within the Essential Levels of Assistance (LEA).

In some specific cases, a co-payment may be required for any additional social assistance services, in accordance with current regional legislation.
